Footprints Carolina provides Person-Centered, culturally responsive quality services and support for individuals with developmental disabilities, by ensuring that:

  • our customers and their families are assisted in directing and managing their own services to the extent they desire, through their involvement in planning services,and an integrated system of support;
  • they are provided with ongoing information and assistance to help them make informed choices regarding service options and providers, to obtain and coordinate services and to address issues;
  • information is maintained regarding their goals, preferences, needs, abilities, health status and available supports for use in developing and updating their plans;
  • crisis plans are developed, implemented and monitored;
  • appropriate linkages are established with their natural supports and other community resources as needed;
  • their wellbeing, health status and the effectiveness of services and supports are monitored objectively on a regular basis; and
  • our customers are assisted in modifying their Person--Centered Plans, as their life circumstances change.
Services offered by Footprints Carolina

Our Services

Footprints Carolina was initially established to exclusively provide case management services for individuals with mental illness, developmental disabilities and related substance abuse concerns. However, with changes in North Carolina's Service Definitions and our expansion of services, we now provide:

  • Targeted Case Management
  • Outpatient Treatment
  • Transitional Housing

More About Our Services

Effective services for people with developmental disabilities involves: evaluating; coordinating; linking; advocating; assessing and monitoring the services they need.

The roles and responsibilities of staff vary according to the needs of the individual, with the customer's full participation in defining their personal goals and services. This also requires our staff to build trusting and respectful relationships, while providing consistent, ongoing support.

Footprints Advisory Councils

Footprints Advisory Council members provide a profound understanding of the issues, and a vital customer and community perspective for the Senior Management Team. Their guidance is valuable to the ongoing planning, monitoring, evaluation and management of our services.

The Advisory Councils are comprised of volunteers, with at least half of the membership being persons with developmental disabilities or their family members. Other members may be advocates, other concerned citizens or persons, who work in these or related fields such as: medicine, public welfare, counseling, criminal justice or vocational rehabilitation.

These are truly committed people, who advocate for a very special cause - quality services for persons with developmental disabilities.

Traumatic Brain Injury (TBI) Support Group

The TBI Support Group meets at the Footprints Corporate office every 3rd Tuesday at 5:30 p.m. The group is assisted by Shirley Moore and Patricia Haithcox, two dedicated professionals with many years of experience working with individuals, who have TBI.

This group, for survivors and families, provides an opportunity to meet with your peers. It also provides access to counseling support, community information, updates on the latest information regarding traumatic brain injury and socialization opportunities. Transportation assistance can be arranged, if needed.
